Monthly Membership Pricing

(effective 12/1/25)

0-19     $30    (with enrolled adult, $60 without)

20-29    $60

30-59    $90

60-79    $120

80+       $155 (includes home visits)

Discounts: 10% family (same address, not applied to oldest member or 0-19), 10% teacher, 10% active military, 5% if full year paid in advance.

Enrollment fee: Twice the monthly rate, maximum $300 per family, due at the initial visit.  Re-enrollment fee $300 per person.

Home visits: 1 free home visit annually, additional home visits are $25 each within Alameda, $50 each in Berkeley and Oakland (limit 20 min drive).  Home visits INCLUDED for 80+.

Vaccines: Cost plus 10% rounded to nearest dollar amount.

Single visit:  I prefer to have an ongoing relationship with patients, but sometimes you just need a single visit - $350 -$600 depending on complexity - email or call to discuss.
